{"identifier":"/us/usc/t42/s254c","title":42,"num":"\u00a7\u202f254c.","heading":"Rural health care services outreach, rural health network development, and small health care provider quality improvement grant programs","text":"\u00a7\u202f254c.\nRural health care services outreach, rural health network development, and small health care provider quality improvement grant programs\n(a)\nPurpose\nThe purpose of this section is to provide grants for expanded delivery of health care services in rural areas, for the planning and implementation of integrated health care networks in rural areas, and for the planning and implementation of small health care provider quality improvement activities.\n(b)\nDefinitions\n(1)\nDirector\nThe term \u201cDirector\u201d means the Director specified in subsection (d).\n(2)\nFederally qualified health center; rural health clinic\nThe terms \u201cFederally qualified health center\u201d and \u201crural health clinic\u201d have the meanings given the terms in\nsection 1395x(aa) of this title\n(3)\nHealth professional shortage area\nThe term \u201chealth professional shortage area\u201d means a health professional shortage area designated under\nsection 254e of this title\n(4)\nMedically underserved community\nThe term \u201cmedically underserved community\u201d has the meaning given the term in\nsection 295p(6) of this title\n(5)\nMedically underserved population\nThe term \u201cmedically underserved population\u201d has the meaning given the term in\nsection 254b(b)(3) of this title\n(c)\nProgram\nThe Secretary shall establish, under\nsection 241 of this title\n(d)\nAdministration\n(1)\nPrograms\nThe rural health care services outreach, rural health network development, and small health care provider quality improvement grant programs established under\nsection 241 of this title\n(2)\nGrants\n(A)\nIn general\nIn carrying out the programs described in paragraph (1), the Director may award grants under subsections (e), (f), and (g) to expand access to, coordinate, and improve the quality of basic health care services, and enhance the delivery of health care, in rural areas.\n(B)\nTypes of grants\nThe Director may award the grants to\u2014\n(i) promote expanded delivery of health care services in rural areas under subsection (e);\n(ii) provide for the planning and implementation of integrated health care networks in rural areas under subsection (f); and\n(iii) provide for the planning and implementation of small health care provider quality improvement activities under subsection (g).\n(e)\nRural health care services outreach grants\n(1)\nGrants\nThe Director may award grants to eligible entities to promote rural health care services outreach by improving and expanding the delivery of health care services to include new and enhanced services in rural areas, through community engagement and evidence-based or innovative, evidence-informed models. The Director may award the grants for periods of not more than 5 years.\n(2)\nEligibility\nTo be eligible to receive a grant under this subsection for a project, an entity shall\u2014\n(A) be an entity with demonstrated experience serving, or the capacity to serve, rural underserved populations;\n(B) represent a consortium composed of members that\u2014\n(i) include 3 or more health care providers; and\n(ii) may be nonprofit or for-profit entities; and\n(C) not previously have received a grant under this subsection for the same or a similar project, unless the entity is proposing to expand the scope of the project or the area that will be served through the project.\n(3)\nApplications\nTo be eligible to receive a grant under this subsection, an eligible entity, in consultation with the appropriate State office of rural health or another appropriate State entity, shall prepare and submit to the Secretary an application, at such time, in such manner, and containing such information as the Secretary may require, including\u2014\n(A) a description of the project that the eligible entity will carry out using the funds provided under the grant;\n(B) a description of the manner in which the project funded under the grant will meet the health care needs of rural underserved populations in the local community or region to be served;\n(C) a description of how the rural underserved populations in the local community or region to be served will be involved in the development and ongoing operations of the project;\n(D) a plan for sustaining the project after Federal support for the project has ended;\n(E) a description of how the project will be evaluated; and\n(F) other such information as the Secretary determines to be appropriate.\n(f)\nRural health network development grants\n(1)\nGrants\n(A)\nIn general\nThe Director may award rural health network development grants to eligible entities to plan, develop, and implement integrated health care networks that collaborate in order to\u2014\n(i) achieve efficiencies;\n(ii) expand access to, coordinate, and improve the quality of basic health care services and associated health outcomes; and\n(iii) strengthen the rural health care system as a whole.\n(B)\nGrant periods\nThe Director may award grants under this subsection for periods of not more than 5 years.\n(2)\nEligibility\nTo be eligible to receive a grant under this subsection, an entity shall\u2014\n(A) be an entity with demonstrated experience serving, or the capacity to serve, rural underserved populations;\n(B) represent a network composed of participants that\u2014\n(i) include 3 or more health care providers; and\n(ii) may be nonprofit or for-profit entities; and\n(C) not previously have received a grant under this subsection (other than a grant for planning activities) for the same or a similar project.\n(3)\nApplications\nTo be eligible to receive a grant under this subsection, an eligible entity, in consultation with the appropriate State office of rural health or another appropriate State entity, shall prepare and submit to the Secretary an application, at such time, in such manner, and containing such information as the Secretary may require, including\u2014\n(A) a description of the project that the eligible entity will carry out using the funds provided under the grant;\n(B) an explanation of the reasons why Federal assistance is required to carry out the project;\n(C) a description of\u2014\n(i) the history of collaborative activities carried out by the participants in the network;\n(ii) the degree to which the participants are ready to integrate their functions; and\n(iii) how the rural underserved populations in the local community or region to be served will benefit from and be involved in the development and ongoing operations of the network;\n(D) a description of how the rural underserved populations in the local community or region to be served will experience increased access to quality health care services across the continuum of care as a result of the integration activities carried out by the network;\n(E) a plan for sustaining the project after Federal support for the project has ended;\n(F) a description of how the project will be evaluated; and\n(G) other such information as the Secretary determines to be appropriate.\n(g)\nSmall health care provider quality improvement grants\n(1)\nGrants\nThe Director may award grants to provide for the planning and implementation of small health care provider quality improvement activities, including activities related to increasing care coordination, enhancing chronic disease management, and improving patient health outcomes. The Director may award the grants for periods of 1 to 5 years.\n(2)\nEligibility\nTo be eligible for a grant under this subsection, an entity shall\u2014\n(A)\n(i) be a rural public or rural nonprofit private health care provider or provider of health care services, such as a critical access hospital or a rural health clinic; or\n(ii) be another rural provider or network of small rural providers identified by the Secretary as a key source of local or regional care; and\n(B) not previously have received a grant under this subsection for the same or a similar project.\n(3)\nApplications\nTo be eligible to receive a grant under this subsection, an eligible entity, in consultation with the appropriate State office of rural health or another appropriate State entity shall prepare and submit to the Secretary an application, at such time, in such manner, and containing such information as the Secretary may require, including\u2014\n(A) a description of the project that the eligible entity will carry out using the funds provided under the grant;\n(B) an explanation of the reasons why Federal assistance is required to carry out the project;\n(C) a description of the manner in which the project funded under the grant will assure continuous quality improvement in the provision of services by the entity;\n(D) a description of how the rural underserved populations in the local community or region to be served will experience increased access to quality health care services across the continuum of care as a result of the activities carried out by the entity;\n(E) a plan for sustaining the project after Federal support for the project has ended;\n(F) a description of how the project will be evaluated; and\n(G) other such information as the Secretary determines to be appropriate.\n(4)\nExpenditures for small health care provider quality improvement grants\nIn awarding a grant under this subsection, the Director shall ensure that the funds made available through the grant will be used to provide services to residents of rural areas. The Director shall award not less than 50 percent of the funds made available under this subsection to providers located in and serving rural areas.\n(h)\nGeneral requirements\n(1)\nProhibited uses of funds\nAn entity that receives a grant under this section may not use funds provided through the grant\u2014\n(A) to build or acquire real property; or\n(B) for construction.\n(2)\nCoordination with other agencies\nThe Secretary shall coordinate activities carried out under grant programs described in this section, to the extent practicable, with Federal and State agencies and nonprofit organizations that are operating similar grant programs, to maximize the effect of public dollars in funding meritorious proposals.\n(3)\nPreference\nIn awarding grants under this section, the Secretary, as appropriate, shall give preference to entities that\u2014\n(A) are located in health professional shortage areas or medically underserved communities, or serve medically underserved populations; The Director may award the grants for periods of 1 to 5 years.","tail":"\n"}],"tail":"\n"}],"tail":"\n"},{"t":"para","id":"/us/usc/t42/s254c/g/2","children":[{"t":"num","text":"(2)"},{"t":"heading","text":"Eligibility"},{"t":"chapeau","text":"To be eligible for a grant under this subsection, an entity shall\u2014"},{"t":"subpara","id":"/us/usc/t42/s254c/g/2/A","children":[{"t":"num","text":"(A)"},{"t":"clause","id":"/us/usc/t42/s254c/g/2/A/i","children":[{"t":"num","text":"(i)"},{"t":"content","text":" be a rural public or rural nonprofit private health care provider or provider of health care services, such as a critical access hospital or a rural health clinic; or","tail":"\n"}],"tail":"\n"},{"t":"clause","id":"/us/usc/t42/s254c/g/2/A/ii","children":[{"t":"num","text":"(ii)"},{"t":"content","text":" be another rural provider or network of small rural providers identified by the Secretary as a key source of local or regional care; and","tail":"\n"}],"tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/2/B","children":[{"t":"num","text":"(B)"},{"t":"content","text":" not previously have received a grant under this subsection for the same or a similar project.","tail":"\n"}],"tail":"\n"}],"tail":"\n"},{"t":"para","id":"/us/usc/t42/s254c/g/3","children":[{"t":"num","text":"(3)"},{"t":"heading","text":"Applications"},{"t":"chapeau","text":"To be eligible to receive a grant under this subsection, an eligible entity, in consultation with the appropriate State office of rural health or another appropriate State entity shall prepare and submit to the Secretary an application, at such time, in such manner, and containing such information as the Secretary may require, including\u2014"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/A","children":[{"t":"num","text":"(A)"},{"t":"content","text":" a description of the project that the eligible entity will carry out using the funds provided under the grant;","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/B","children":[{"t":"num","text":"(B)"},{"t":"content","text":" an explanation of the reasons why Federal assistance is required to carry out the project;","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/C","children":[{"t":"num","text":"(C)"},{"t":"content","text":" a description of the manner in which the project funded under the grant will assure continuous quality improvement in the provision of services by the entity;","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/D","children":[{"t":"num","text":"(D)"},{"t":"content","text":" a description of how the rural underserved populations in the local community or region to be served will experience increased access to quality health care services across the continuum of care as a result of the activities carried out by the entity;","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/E","children":[{"t":"num","text":"(E)"},{"t":"content","text":" a plan for sustaining the project after Federal support for the project has ended;","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/F","children":[{"t":"num","text":"(F)"},{"t":"content","text":" a description of how the project will be evaluated; and","tail":"\n"}],"tail":"\n"},{"t":"subpara","id":"/us/usc/t42/s254c/g/3/G","children":[{"t":"num","text":"(G)"},{"t":"content","text":" other such information as the Secretary determines to be appropriate.","tail":"\n"}],"tail":"\n"}],"tail":"\n"},{"t":"para","id":"/us/usc/t42/s254c/g/4","children":[{"t":"num","text":"(4)"},{"t":"heading","text":"Expenditures for small health care provider quality improvement grants"},{"t":"content","children":[{"t":"p","text":"In awarding a grant under this subsection, the Director shall ensure that the funds made available through the grant will be used to provide services to residents of rural areas.
